Wafarin dosage

Hi All
I need some advice . Almost three years post AVR surgery and for all these years my INR has been above 2 and dosage has be quite fixed I.e. 4 mg
Since last week my INR has been around 1.8 and I had to increase the dosage to 5 mg and some times take1 mg random dose to keep above 2. I haven’t changed my diet nor I’m doing anything different . Any thoughts/ suggestion. I’ ll be going to coagulation clinic to calibrate my device. I also plan to do INR by doing blood test. However could anyone share their experience ? Any suggestion
I also want to discuss an example
Eg. If a patient who has an INR 2 and takes his usual dose what is the assurance from the time the dose was taken till the time of their next dose their INR stays in range ?it could drop or increase ..
in my case today I tested 1.8 then took 5 mg and then tested in few hours the INR increased to 1.9 . Then I tested again in an hour the INR went to 1.8 . I was worried and I took 1 mg to top up ? I am worried now . Pls help

how far above 2? 2.1 or 2.8 ... do you have average and standard deviations?

I haven’t changed my diet nor I’m doing anything different .

noted, have you changed brand of warfarin?

I don't think anyone wants to send you a birthday card, so knowing the actual year would help far more than knowing the month (let alone the day). Are you 35, 55, 75 years old?

Also what valve you had ... Putting such details in there saves people who wish to help from spending a lot of extra time on searching your prior posts.
