I haven't written for a while but really need some opinions. So I thank you in advance for your thoughts.

Thank you so much for your thoughts on the subject. This is so out of the blue. I am sure most of you remember that I am on the national database for 9 months now awaiting to climb the ladder for a double lung transplant. Still don't need oxygen and seem to be holding my own.

I am not getting better and have gone a little in 6 min. walk test, stayed lateral in others and down a tad in some areas.

So this new development is out of the blue. I mean I new about the Aortic Valve reg. but did not know about the the blocked artery. My last right and left heart cath. Oct. 2002 showed 10-20% stenos is of the coronary artery but now as of 3 days ago my last right and left heart cath shows a little over 80% blockage of that artery. If this was in a different place and I wasn't so small they would automatically do a stint or angioplasty. My Cardiologist called in the the Heart/Lung transplant Surgeon and they both agreed that it is much to dangerous to do this on me so the only way to go would be bypass surgery. Obviously they would go ahead since I have mod. Aortic reg. To do that surgery at the same time.

Because they feel this is a complicated high risk case because of my valve reg. And my Pulmonary Fibrosis the surgeon wanted to talk to my lung transplant team, Cardiologist, my main primary care physician and everyone else under the sun.

So we now have an appt. to meet with the surgeon Tues. morning at 10:30 CST. To hear what the options are. This impending surgery which is scary.

I am scared that they may have me between a rock and and hard place. If I were to wait I get older and weaker and take the chance that later they might not be able to do the heart surgery because of my lungs. The other issue is with Valve being only moderate the lung transplant people are okay with that as long as I have my test every 6 months but now with coronary artery diseases it would very likely make me ineligible for the transplant at all, meaning they would say fix the heart and she can stay on the list, don't and she is off. So I may not have a choice.

From those of you that know please tell me what your opinion is of what they will decide based on the 2 years you have known me and this new information. I know that none of you know for sure but waiting is driving me nuts. They have me on Nitro pills and a Nitro patch for now till Tuesday.

If they do have to do the bypass which the surgeon (who is both and heart and lung transplant doctor) said if we go into do the bypass having moderate valve damage and just doing a Phen Phen woman a little older than me he would do both at once, no use cutting me twice when we can kill two birds with one stone.

We were supposed to go to Prague and Vienna and Austria (my husbands cousin just got promoted through 3M) leaving March 5th returning March 19th. The surgeon said that is out of the question he is not at all comfortable waiting that long and if something happens in another country it is bad, not like going to Florida. So it sounds like if they do it, they are going to do it right away.

Ok that is all the pity me for one night. Thank you all and if you learn anything else please please email me. I appreciate your friendship. I just want to hear thoughts please.

I don't really have any advice for you, but wanted to let you know that you will be included in our prayers. I am SO sorry about this set back. I frankly am shocked that the artery occluded so badly, so quickly. Let God be your shining light on THIS path, as he has on your road thus far.

Not only you but both the Cardiologist and the surgeon were both surprised that the artery occluded so badly, so quickly! They said that was very very unusual especially when my other arteries were ok? Interesting thank you for your prayers I appreciate. We meet with the surgeon 10:30 tomorrow morning!

Lisa, I had to read that letter twice. It makes me shudder and wish I were God. (maybe that feeling in the young makes Drs.)

To be blunt I am glad that no one close to me is in your circumstances, I don't know how I would be able to deal with it other than to be there for them. As close as I have been to one is a friend has a son that married a girl in Virginia and she had to have a double lung transplant. Last I heard all is O.K.(two years now).

As far as what you are facing, I see it as if you are on a raft on the Niagara river flowing toward the falls. You must jump into the water and swim to shore to avoid the falls. You might not make it. You know you are allright on the raft instead of in the water, but there is a point that if you stay on the safety of the raft it will be too late to swim to shore. Now you will face impending doom traded for a few minutes of safety.


That seems to be a rapid closing on that artery. I believe your surgeon will want to hit the valve and artery(s) in order to get you in top shape to handle the transplant. Step, by step, by step to a comletely repaired and well LISA. :)

Lisa, I don't know enough about this to comment, other than that you're in my prayers and thoughts. I'm so sorry to hear this, but you know, at least they found it before it was totally blocked.

Good luck tomorrow. I'm glad you have good docs behind you.

Mamsram I love your analogy you made me smile and laugh! Georgia thank you for your prayers.

I will definately let you know how it all turns out after we see the surgeon tomorrow.

Do any of you think there is a chance they may choose not to do any surgery?

They called several doctors down to look at the artery while I was still in the Cath. they all agreed it was much to narrow and putting in a stint or the angoplasty was way way to dangerous must do a bypass or leave it alone and watch, but I do remember the surgeon saying 80% or better I don't think so. ???
 
Lisa, I have no wisdom but I do indeed have you included in my prayers. Hang in there girl! And come to us and whine anytime you want to.

What part of Dallas do you live in? I grew up in Dallas (Lake Highlands High School area). My parents still live in that neighborhood. My dad has pulmonary fibrosis, goes to a Dr. Millard (sp?) at Baylor.
While I was in ICU at Baylor after my OHS, the woman in the next bed was up and about, very good for a heart surgery patient, I thought. Later learned she was a heart-double lung transplant patient. Don't know how long post-op she was.

Please let us know tomorrow what your surgeon says.
 
I don't have much to add. What I don't get is why, if the heart is in such a bad way, don't they do a combined heart and lung transplant? I would think that given the circumstances, that would push you to the top of the transplant list or very near it.

Sorry Lisa, I don't know how to weigh in on this one. You can be sure that I pray for a great outcome no matter what is chosen. ;)

Sorry didn't have an email to reply back to but. My lung transplant doctor is Dr. Ken Auloose at Baylor Downtown and I have 8-0 months accrued and I am on the national database.

The heart surgey would be at Medical City Dallas he is a heart/Lung surgeon.

My Pulm. Fib. is not bed enough to be transplanted but I am on the double list for the national database.

The heart would be for the aortic valve and for the blocked artery to keep me n the list and bcasue there is 80% blockage Thank you for your prayers.

The reason is my lungs are not yet ready for the transplant. So I definately want to put that off as long as possible. The feeling is that since I did have the open lung biopsy 2 years ago and have been offically DX. with PF I made the national database and have been accepted. They took me with the valve problem with the condition that I am check with a right and left heart cath every 6 months and a kidney glofil test.

So that is why they aren't going to do both at once. I am not even on oxygen yet. My left lung is the wrost. My dlco is 33% and there is some Fibrosis started on the right lung but as of today I am not bad enough to be transplanted. But not with the complication of not just the valve but the blocked artery as well they may say "fix the heart to keep her on the list" or she is off? Just don't know how that works.

Just read your post and want to say that I don't hear "pity me" but rather a really strong gal who is frustated and justifiably frightened by her prospects. You sound very much like a lady who can handle whatever life throws at her, I just wish you didn't have quite so much on your plate. We're all here when you need to unload or want a little support. I am praying for you. Yes, you sure have some rocky roads ahead of you. I'm sure that you are scared, but you seem to be knowledgable and have your head on straight. That will help you get through this.

As to what to do? It is my hope and prayer that your group of doctors can determine that quickly. You may need to be prepared for them to give you some options and have you decide.

My experience at Medical City Dallas was fantastic, and I would recommend it to anyone. I travelled from Allen to be with Dr. Mack at his home base. I was coached that the surgical staff and the teamwork are very important. My question is....Why would surgery be there if Ausloose is at Baylor? Baylor is also a great place. Can he bring his regular crew with him? Seems like a fisherman would be more effective in his own lake with his own boat.

My suggestion is to write down all of the questions and concerns as they come to you. Bring the list with you tomorrow. That way you don't have to rely on your memory during the consult.

Thanks so much for your thoughtful suggestions. Hoscktom, Auloose probably would not be there. This is only for the Heart the bypass on artery and the valve replacement or repair. Auloose is my lung transplant Dr. at Baylor but my heart transplant surgeon is Dr. Mitch Magee at Medical City Dallas. Does that make more sense now?

What kinds of options do you think they could throw at us? Just to give me an idea?

Dr. Magee is in the same group as my Dr. Mack. You should be in great hands.

I really don't know what they might throw at you, but it may come down to choice of which risk to take and/or choice of lifestyle.

Color schemes and decorating are not my forte. My wife makes the selections, and I help. A couple that wallpapers together stays together.

I know we all have had our problems, but after reading yours I feel like mine were small to what you are going through. My thoughts and prayers are with you! You and you alone can make the right decisions for you. But I feel like God gave me a second chance for a reason. Good luck with whatever you decide. You are too young and beautiful to have theses happening to you . When I saw your picture I thought of my daughters.

Hey, Lisa - we missed you.

Sounds like you are on a road uphill. I am so sorry, but if your doctors think this might negate the possibility of lung transplant, the heart surgery must be your best bet. They are so advanced these days that they will have you on very special watch all the time. It sounds so complicated and your case definitely is. Please keep us posted - and you know, from your past experiences here, that you are on many hearts for prayers. Love Ann
