I'm trying real hard to keep a clear head about this, so tell me if this makes sence:
If terodac is correct about dropping a full point for every day that is skipped, and I skip/hold until Saturday, my INR might be around 2.5 - ah much better place to be in my doctors eyes then I could start the alternating of the 5 and 6 mg and retest on Thursday.

Maybe this is worth a try?

Hi Freddie,
I am no expert, not even close, but from my own experience and having just spent two nights in the hospital, I say trust the forums! My dr. had my hold one night @ 4.1 and I dropped to the 1's. Of course, my cardio says this is just strange, but I wouldn't panic over 4.9. I was up to 6.0 at one point in the spring and I think only dropped a dose one night... then I crashed. But again, that is me, so I wouldn't base anything off it... but the forums have never steered me wrong, can't say the same for the docs. Good luck.

Halley thank-you. I am so confused - don't know which way to turn. Part of me wants to hold for tonight, the other part of me says take 3mg tonight. Then my mind is saying just screw it.
Thank-you for sincerity.

Freddie, I know just how you feel, my INR confuses the heck out of me all the time. I would really take the 3 and eat some spinach ;)... then relax! I don't know much, but I do know if I was in your situation, this is what I would do.

oh yea, the doc did mention that if this plan didn't work, we'd go talk to some specialist to see if we were missing something. Since 6mg was my normal dose for over a year and is now getting me close to 5.0 something is else maybe going on. The only thing I can think of is having Chinese food once a week at the curling rink - which ended in March. Could Chinese food have that big effect on ones INR? But on the other hand, my INR didn't go collie-wompa until May 21.
Nothing makes sence.

Yea, I know that and I questioned my doc on that. She figured it should still show a drop, if I was to hold for 2 days.
Maybe the reason she wants me to hold for 48 hours is to start over with my dosage. uhmmm, if I was to get my INR low enough then we (she & I) could slowly bring me back into range..........but would that be safe??? What am I saying, I was at 2.0 and I'm still here, so maybe that's her plan.

Freddie i'd suggest no hold and hopefully the tornado storms
havenot shaken her thinking pattern too much.....omgosh what silly
silly reasoning on her part, for sure i wouldnot trust her judgement on your
inr instructions.
You have more patience than me even without a Dr.like that and no other
doc id have stopped going or had one dam good argument with her already
she would likely thrown me out as her patient, but id believe its do to her not being able to stabilize warfarin and so many like her in our world, that cannot get with the program of coumadin.

I don't know but i'd trust vr before anything or anyone else, just my opinion
and i know any changes in my dosage etc takes 3 days for to show up,at least 3 days.
Maybe your getting too much advise from your doc,and I feel she means well
but just isnt good at maintaining proper use of coumadin, neither is my doc

Frustrating and i'm sending you (((HUGS))) it gets better, really it does

wow alot of great info here for you, took me while to read it all.

Freddie (I'm back - had to go run rehearsal)
Our advice and the calculator are basically the same. It says hold 0 - 2 doses. Most of us that have been on Coumadin for a long time know that a 2 day hold for a 4.9 is outrageous and the calculator (and protocol) only suggests it if, for some strange reason, there is bleeding going on.

As Jim pointed out - you Dr. and the way she freaks out over an INR that is a point or 2 over your range is what is causing the problem.

Don't forget, some people here have a range of 3 - 4. Your doctor would probably have them live their life in the hospital so as to catch any horrible bleed as it starts ;)

It is extremely dangerous to think that going low and starting over again is a good plan. I would rather be 6.0 than 1.8. (And I've been both several times).

If you take 3 tonight and 3 tomorrow night, your test on Saturday should be lower. But for heaven's sake - do not let your doctor convince you to hold if it's over 3.

But after looking at your dose history - it looks like 5 mg/day (35/week) is your sweet spot.

Please tell your doctor that there can be as much as a .3 variance in any test. So if she's freaking over a 3.5 or 3.6 - it could very well be a 3.2 or 3.3. (and yes - a 3.8, or 3.9) She is micromanaging you into one heck of a yo yo.

She's talking about you seeing a "specialist". Please show her your weekly INR's and doses and tell her that she is the reason your INR is going all over the place. (I know you won't, but think about it. ;))
