It varies right now he takes 1.5 mg of coumadin mon - thurs and 1mg fri - sunday. he takes 2 shots every day one AM and one PM
 
Those drug interactions are probably why he's on such a low dose of warfarin. the fluconazole is a big one.

And the lovenox shots are probably because his INR is still lower than they want it. Once his INR is stable they should discontinue those.

I'd agree with Ross, increase to 1.5 mg warfarin x 7

But the home machine will get an H error code if he's taking lovenox - it doesn't work right when you've got heparin in the system. So until he gets off the lovenox you can't use the self-tester, it has to be a lab draw.

I never had that happen when I was taking Lovenox.

If I were his doctor, I'd take him to 1.5mg per day x 7 days and then test him. If he's over 2.0 then, the Lovenox could be discontinued.
 
he is not curently taking the shot, they have now taken him off of that so hopefully we will be able to keep him on track for a while.......
 
Just an update, he had blood work done on Monday and they finally called with the results on Wed, his INR was 2.07 and they said that's where they want it so currently he is to take 1.5 of coumadin daily! Now if we can just keep that straight and get that machine! We called yesterday and were told it had been sent from somewhere and was on it's way to his Dr. office in Richmond so hopefully they will send it back out the day they get it and send it to him. In my opinion there is no point in doing the test on Monday if we don't know what it was until Wed!
