Hello there,
My elder bro has been detected a severe aortic steno sis . He has been suggested a surgery . While doing a my home work on net , finding out information , I came to know about heart valve replacement without surgery . I would like to ask, if any one has came across this surgery/reference/detail info/review? IF y , plz post . I would be very thankful .
Also my bro is in india Mumbai. Is this surgery available in india?

Hello Aniruddha and welcome to the forums.

The surgery that your speaking of is used only for those that cannot endure a full heart open heart surgery and the valve is replaced by catheter. As far as I know, it's still only used as a last resort for very sick patients.

Not sure what's going on with your older brother. Can you give us more information?

Hi they are called percutaneous valve replacement, as Ross said in the US it is still in clinical trials, and they are only being done on poeple who can't have open heart surgery.

I have also talked to a surgeon in Miami who did one in the tricuspid postion on a little boy who had previously had a valve replacement.

More over , actually I am kind of going through a confused state :confused:. Some docs saying that angioplasty will also b a solution for this aortic stenosis . but it will last till 10 yrs from now den again the same pbm would come. And some docs saying that surgery i.e. AVR is the only solution .
Not understanding what to do ? Attaching a snap of report . Can any could HONESTLY suggest , what would be the extract of report?

What did he have done in 1999? and the first time? did they just open the aorta in the cath lab with balloons? (How old is your brother? my 21 year old son had a ballon dialtion in 88 in the cath lab.
or replace part of it in surgery? I see he has a bicuspid valve, , so I that rules out the valve by cath in the US trials, I don't know about Indi, but did they say he NEEDs his vsalve replaced or "just" a section of his aorta? Are they saying if they use a balloon to upen the aorta it will come back in 10 years OR if they replace the valve with a tissue valve it will come back in 10 years? IF they opened the aortic stenosis, in 88 and 99 and he got 10 years between caths, I'm guessing, that is what they are talking about would only last 10 years. Justin has had alot of caths (I lost count) and 5 OHS and IF he could get a repair/ balloon that would last 10 years, instead of OHS, I think he would take it, since the recovery from caths is SOOO much easier.

In past two decades , i.e for 89 and 99 , a surgery had done using catheter . in 89 , there was a baloon angeoplasty for widening his LV vein . And in 99 they did a angioplasty for widening Aortic valve. No open heart surgery till date.
So what I was confused at , observing conflicts in couple of docs advice . Some says balloon angioplasty would help him to do good will next 10 yrs , some says that AVR is must this time. I hope you getting what I mean .
 
There's one thing I saw in the report that might need attention that I'm not sure anyone's mentioned yet:

Post stenotic dilatation of the aorta. This means that his aorta is becoming too big (aneurysm on the horizon)- which eventually will be a big deal in itself. He may need this resected at some point, maybe now if it's large enough. Albeit, I don't see any measurements of the aorta, and a doctor's definition of dilatation is fairly subjective.

How old is your older brother? What is the width of his ascending aorta?

Personally, such thing is not practised in India. I went to AIIMS and they suggested that only OHS is the answer for now, what will happen in future is not known.

So, even if you find someone doing this, please ensure that he has experience and is not doing as experiment on your bro.

I am not good with nos. so leave that for Ross and others to interpret, but PSG =91mmhg is pretty high. Anything above 75 is in for operation and even sometime less if patient has symptoms, that's what the docs suggested.
