Full Dental Extractions Fully Anticoagulated?

My dentist would not extract one wisdon tooth unless my INR was lowered to 2.0. I did that, but I think that It would have been ok if I did not adjust my INR. I did not want to argue with the dentist who said he had a lot of experience ( oh yeah).

Mine didn't listen either and the Lovenox was a cause for my major bleeding event afterwards. Some people will never never learn.

Hello Ross. i just had 3 impacted wisdom teeth taken out. My cadiologist had me go off cumadin and onto lovenox 4 days prior to surgery. Everything went fine (except looking like a chipmunk). I was told by the cardiologist to go right back on Cumadin (doubled my dose to 10mg) the day after surgery. everything went fine for about three days then the bleeding started and would not stop. They then took me off cumadin and kept me on lovenox. It's now been about three weeks since the surgery and last week I started up again on cumadin. This past Saturday, the bleeding started again in one of the extractions. The cumadin lab keeps giving me bad advise, the cardiologist says listen to the cumadin lab, and the oral surgeon says it's a cumadin problem. Are you having bleeding complications? Thanks jim

I have no personal experience - but from what I've read it's the Lovenox that causes the bleeding problems more than the Coumadin. Unfortunately if you aren't in-range when you start to bleed, then they can't exactly take you off the Lovenox and leave you unprotected for any length of time.

Thanks. I got a call this morning and my cumadin is right around 2. They increased my dosage last friday so it should be kicking in soon. The cardiologist said the range is supposed to be 2.5-3.5 but they said I could stop the lovenox. Now, hopefully the bleeding stops and my gums start to heal. Three weeks eating of mush is a great diet. I lost about 10 pounds! A diet I don't reccomend.

Tell me about it. The food processor is my new best friend. I've thrown subs, pizza, heck you name it, into it. Can't be good for the blade but oh well. I'm really disturbed because on New Years Eve, I like to eat fresh veggie trays and now, I'll have to steam them enough to be able to handle it and that just isn't the same.

Yum, yum...pizza in a blender. I can taste it now. My wife would take the leftovers, throw them into the blender with a little water and presto, DINNER!The oral surgeon said The cumadin lab should have kept me off for a couple days after the surgery, but no, they called and said double dose up on the cumadin to get back on tract! I felt like a trainwreck!

Your oral surgeon is wrong and so is your Coumadin clinic.
Fact of the matter is, if you were in range, they should never have taken you off in the first place! Mine wouldn't listen either and that Lovenox caused me two emergency room visits. Why can't these doctors learn? Doubling your dose is like stepping on the accelerator, then they'll want to slam on the brakes because you went too high.jimbo

I too ended up in the ER twice with massive bleeding. It was Dec 8th when I had the extractions, and my gums are starting to heal up. I still have some bleeding though. The oral surgeon said it usually takes about a week. I think were the "ginny pigs". Do you know how long it take the lovenox to leave your body? Maybe we both should come up with a proven diet book! "How to eat mush and lose 10 pounds in a week".

My understanding is that Lovenox is 'mostly gone' after 12 hours, hence the 2X daily dose, and should be Totally Gone after 24 hours (which is the usual stopping point, i.e. 24 hours before Surgery).
There seems to be some 'variation' on when to resume Lovenox after Surgery with some Doctor's starting up that night and others waiting 24 hours (or more) depending on their assessment of the Bleeding Risk vs. Stroke Risk.

My "expert Cumadin lab" had me double up my cumadin (5mg to 10mg) and one lovenox shot the night of the surgery.

Doubling the dose isn't a big issue, it's just stupid. Unless your renal function is impaired, Lovenox should be completely out of your system in 24 hours and it's real effectiveness wearing off after 12.
I had to go unproctected for a couple days to get the bleeding to stop. They restitched my mouth twice before getting it to quit. I just had a blood test 2 week ago and Im still low on Red cells. They really should have given me at least a pint of blood before leaving the hospital the last time.
