Still seems like a lot to me when people stop Warfarin completely for that long, even with Heparin. I think if I ever had a similar situation I’d try doing it like Chuck C, without bridging with heparin, & just lowering your warfarin dose and testing a lot.

Most of the ones I’ve met don’t have any idea at all.

same here, which is why I dumped them back in early 2013 and went out on my own. Instead of being 70% in range I'm 97% in range and I have full control over what I choose to do.
