I said 5.1 or better, anyone closer?

Whatever you do, don't let them give you Vit K to rapid reduce unless your bleeding!

Yup. While the 7.5 dose looks like overkill (poor choice of words), it was a great opportunity to make the last giant step off the "bridge" once the Lovenox was stopped.
 
Ya'll might be interested to know (well I'll tell you anyway :rolleyes: ). My orders were to hold a dose, then take 1.25 mg, then 2.5 mg. Sure enough, 3 days later my INR was 2.0. My next orders were to resume the most recent normal dose. Scrap the charts and go back to what works for me. Viola...after a week, I was 2.6 today. Hooray! :)

My abosrption rate and dissipation rate is much quicker than it used to be. I take the same dose (well very close), but it goes through me faster.
 
After I have records on someone for a year or two and they need a dosage change, I try to find one that worked for them previously. It is much better than playing with a calculator and doing mental gymnastics. If you need have your dose tweaked a little, it usually works.

Good point.
It's just as important to record doses and changes made to bring INRs back in line after high/low swings. I've recorded my INRs since I began testing, but was not great at recording doses. I am now.
When I measure out my tabs on Wednesday for the following week, I write them down in my INR record book.
I never thought of using the INR/dosing records like you mention. I will now!
 
Al, yes, the wisdom of going back to a previous dose was immediately obvious and welcomed.

Marsha, I recorded my bridge-escape doses this time as well. Great minds think alike. Must be the Texas air! :D
