I have read on Al's website but haven't ordered the dosing guide, I will get one Asap.. I think I'll be a lot better when I'm managing myself, as long as I still have you guys LOL...... I haven't tried the Tylenol arthritis, I'm sitting here with a heating pad rubbing it up and down my arm right now! I'm really wondering about tomorrow night now, should I do the 2mg like she said or take 4mg like she has had me do tonight and last night????? I took the 4mg again tonight so that is 2 nights on 4mg....I go see the "Trust Me" nurse monday morning.... I am glad they got the Lovenox doseage correct!! I feel so much better knowing I am safe......

You have to stop the yo-yo. Stick with the same dosage for at least 5 days before you retest. If that means taking 4mg tonite and Sunday night do it.
AND if your dosage needs to be readjusted come Monday, we'll all deal with that on Monday.

Part 2 - try to follow this (it may make head head spin LOL)
if you stay on the 4mg, that will be 4 days on the same dosage correct? Therefore, when you get tested on Monday, the first 4mg you took (4 days ago) is well into your system and you should get a more stable reading.

I will be praying for the doctors to get this right for you. Like many said here looks like someone screwed up and does not look like they really know what they are doing, but God will see you through it and will bring someone to get this corrected for you.

To protect yourself, you will need to follow the instructions you received from the Coumadin Clinic Nurse. That way if you are not in-range it is on HER back and they cannot claim that you are a "non-compliant patient".
IF you take 4 mg when the Nurse said to take 2 YOU may be considered non-compliant and any problem may be considered to be YOUR fault and she/they may be 'off the hook'.

IF you show any signs of bleeding, you should go to a hospital and request an immediate Lab Draw for INR or present yourself to the ER. Any change in medication will then be the responsibility of the Doctor you saw.

On Monday, I would recommend asking the Clinic Nurse what her Dosing Plan is for the WEEK which is how anticoagulation is typically scheduled. If you do not agree with her recommendation, you can offer your suggestion for consideration and discussion. The Good Ones will usually consider your suggestion or give their reasons if they disagree. Either way, Get the Final Schedule in WRITING from the Nurse. (My Clinic always gives every patient a card or printout with the 7 day schedule from that date forward). I'm thinking you should get a second opinion / confirmation of this plan from the Clinic Manager or your Cardiologist if you are skeptical.

It would be useful to write down the Dose you took Every Day (by Calendar Date), who ordered that dose, and the INR from each test alongside each date, and continue to do this daily. You may want to give a copy of this to your Cardiologist or his nurse while you are there on Monday. Be sure to keep your original copy of all orders.

Disclaimer: The above are strictly my NON-professional Opinions.

Stick with her plan, for now. Being that we still aren't 100% about your sensitivity, lets not push it. If you've barely moved up the scale at all on these doses for the next test, then I, myself will be 100% sure that your not sensitive like first thought.

Don't give them any reason right now to deny you the ability to self test!

I agree, do what the nurse says for now. If you're still too low on Monday - yes - ask that a weekly dosing schedule be given - none of this dose one night -test the next day stuff.

Not to scare you - but an FYI - if for some reason you start to get bleeding (like nose bleed or something) and your INR is still low or in range - it's the Lovenox - don't let them tell you it's the Coumadin. If it starts to rain while the sun shines - a lot of people will tell you it's because of Coumadin. ;)
