This risk of bleeding at 3.8 compared to 3.5 is so small that you could not even calculate it. When you have a known factor (diarrhea) that caused the INR to go high and that factor has been removed the warfarin dose should not be changed at this level. If the dose had been lowered and the INR went too low you would be at risk of clotting. It boils dopwn to what would you rather have, a bloody nose or another valve replacement surgery.
