as I understood it yes, that' right. The transition onto Warfarin is the time of 'close monitor'.

MIMS has this to say:

Initial dose. Marevan therapy is commonly started above anticipated maintenance dosage levels. A commonly used regimen for Marevan is 10 mg/day for two to four days; with daily dosage adjustments based on the results of PT determinations.

And

Special risk patients. Marevan is a narrow therapeutic range (index) drug and caution should be observed when warfarin is administered to certain patients, such as the elderly or debilitated, or when administered in any situation or physical condition where added risk of haemorrhage is present.

I wonder if that has something to do with it? However bear in mind a few things:

• if your stroke risk is 1% then its raised to 1.7%, what is your risk before
• look at the target group who are on warfarin. Many more are on it who are not valve patients and are generally ill or frail (othwise written as co-morbidities). Those of us who have valves at younger ages are likely to have far less problems than an 80Y-old with DVT ...

Yes really ! Here's a link to the hospital's list of urologists, he's Mr John **** (they're Mr's even though they're Dr's 'cos they're surgeons in the UK): http://newvictoriahospital.co.uk/Consultants/urology.html I wonder if he decided to go into urology because of his surname or becasue he was just interested in that area of medicine. But blimey, we can't be the first people to notice his surname, so many people must wonder…..I wonder how many people dare ask him about it ?
