It's been many years since I was a member of this site. I was Lisa in Katy then. Anyway, I'm doing good, but decided to come back due to my recent trouble with finding XS strips. I had no idea that eBay wouldn't be selling them anymore, so had a bit of a panic. Anyway, I'm back because I want to make sure I know these things before it affects me. Almost 26 years post surgery and still doing great.

Second recent issue is that I recently started Nexlizet, which reportedly has a minor interaction with Warfarin, but for me, I went from in range for about 6 months to 4.7 within a week of starting it. I home test, but report my results to the Coumadin Clinic where my Cardiologist practices. They're really good about letting me do my thing and it makes my doctor feel better. I called it in and let them know I'd be taking a half dose for 2 days (I don't like to hold a dose even though I know it's the same thing), then going back to my normal dose and testing again in a week. My normal dose was 7.5 mg 5 days a week and 4.75 mg 2 days a week. It was still high. Now I'm taking 7.5 mg 3 days a week and 4.75 mg 4 days a week. I recently did the math and figured out it's almost the same as 6 mg per day, so I might be changing my standard dose in the future if I stay at this dose. I just got a 90-day refill, so it won't be soon!

well basically you need to adjust your dose down to account for the interaction. If you want a more reliable method which gives you suggestions I can show you what I do, but basically just decrease the dose to account for it, and perhaps I'd start by dropping the dose by 10% ... with more data I can give you better answers.

that's a pretty wild swing and I would urge you to move towards a more consistent daily dose like maybe 5mg per day ... given that we need to reduce your dose anyway. Its not unlikely that because of such a wild dose alternation that you could be 4.1 on one test day and 4.7 on another.

That's possible, Pellicle, but I test on different days to try to catch it and since I've been on this dose for about a month, I haven't caught any wild swings. 26 years on Warfarin. I know what I'm doing.
