K MY FAMILY DOC HAS CALLED AND DOESN'T KNOW FOR SURE HES SCARED HE DOESN'T CREATE A BLEED.

tHESE ARE WHAT HES RECOMMENDING

70 MG A SHOT OF LOVENEX AND TELLING ME TO TAKE 7 MG WARFARIN WITH THAT SHOT!!!!!!!!

MY WEEKLY AMOUNT OF WARFARIN I TAKE IS IS 6 AND 7 TOTAL FOR

WEEKLY IS 48 MG WITHOUT ANY ADJUSTMENT IF NEEDED.


i,VE NOT TAKEN ANYTHING YET AS HONESTLY HES AFRAID HE MAY CAUSE A BLEED MY INR THIS MORNING WAS 1.2

CAN SOMEONE PLEASE HELP ME IN RECOMMENDING WHICH WAY I GO

OR SHOULD I FOLLOW HIS INSTRUCTION,HE TOLD ME TO CHECK WITH

YOU GUYS IF ANY HELP I COULD USE WOULD BE OFFERED AND FOR ME TO WAIT TO TAKE THE SHOT AT 7 PM AS EDMONTON MAY STILL POSSIBLY DO A SURGERY TOMORROW MAYBE.

THANKS SO MUCH ANY SUGGESTIONS CUS I DONT KNOW EITHER.

Gosh golly gee whiz - I wish I could help with your question.
If you were to take 5mg right now.....would that be so bad? It may help your levels a bit for the time being. As for the shot........best wait for the pro's to answer.

can't see why i just can't build warfarin up slowly .....why even the shot and my doc asked this too?

Deb,

Lovenox does not effect your INR. The lovenox dosing is based on body weight. http://www.lovenox.com/hcp/dosingAdministration/weightBasedDosingChart.aspx

If your range is 2 - 3 you are out of range at 1.2. The lovenox is your bridge to prevent clots while lowering and discontinuing warfarin, and starting it back up to get back in range. You should be ok as this is what I did after my surgery before I reached my therapuetic level, as long as your doctor has dosed the lovenox correctly.

If your at 1.2, that is the same as having no warfarin in you at all. Taking 7 mg now isn't going to hurt a thing. It's going to take three days for it to show up anyhow. Lovenox shot will protect you for 12 hours and that's all. Unless your going to start the Coumadin back up altogether and get therapeutic again, which seems to be the path your doctor is taking, then follow his directions. That's all you can do at this point.

Just so I understand this mess; Zipper IS to have the shot and proceed with her regular dosage of warfarin

I'm running with the idea that her doctor is going to start her back up fully on Coumadin and get her therapeutic once again, meaning she'll take her 7 tonight 6 tomorrow etc until therapeutic. Until she is at least 2.0, she should have an injection of Lovenox every 12 hours.
 
we just dont know how many injections of lovenex i will need.

1st one at 7 or there at tonight.

I echo exactly what Ross said Deb. Take your Lovenox injections as instructed by your doctor, every 12-hrs. Lovenox is to keep you from clotting until you get into therapeutic range.

Don't sweat it too much; you're gonna be fine!!! Relax! :)

Deb, What do you mean cancelled? The surgery or just the meds? I hope you are not having to wait again. I hope everything is all right and you have to go ahead.

Deb, sorry to hear about the cancellation.
Take the Lovenox shot tonight, and start your Coumadin/warfarin at least with your regular dose.
Like Ross said, the pill won't show in your blood INR for another 2-3 days. So take the Lovenox too.
 
Your Doctor will need to write a prescription for X number of pre-loaded syringes which come in 30, 40, 60, 80, and 100 mg doses. Does your insurance cover Lovenox? (It can be really expensive without insurance)

It is best to get ALL of your required doses on ONE prescription to avoid multiple co-pays -(at least that's how it works in the USA). Has your Doctor given you a prescription, or called one in to your pharmacy?

It will most likely take at least 4 days for your INR to become theraputic (maybe more) so you will need at least 8 syringes of 80 mg, leaving 10 mg NOT injected each time. The BEST way to NOT overshoot your INR range is to resume your normal Coumadin dosing schedule (i.e. NO Loading Doses). (Lovenox does NOT interact with INR readings)

Dosing is based on weight with 100 mg for 220 lbs so 70 mg would be appropriate for someone weighing around 154 lbs. Is that close to your weight?

It would be REALLY GOOD if someone (Doctor, Certified Coumadin Clinic Nurse, or Certified Anticoagulation Pharmacist) can instruct you on self administering the injections.

I have no advice for you since I'm not on Coumadin and know zip about it.

I am sorry to hear your surgery was cancelled. I don't know how you do it but you will get through this. You're tough.
