Hi all

Recall a few different versions of charting formats posted in the past.

Would greatly appreciate it if someone could post them again.
New doctors are requiring a fax once a month to chart my levels.

INR Chart

Dick, I record in a booklet printed by Dupont which they will send on request. I also made one on my computer which I don't like quite as much.

Marty, Gina, I emailed it to both of you. It is in Excel. It is a fax form I use to report my INR's and dosages to my Cardiologist. As you can see by the chart, I have been using this form for about a year. Works for us.
