Didn't one our members state the first adopters of anticoagulants were to dose until they saw blood in their urine and then back off a bit?

I've heard this too. I never did it this way. In the '60s, the test wasn't as accurate as today.....but I regulated my "pro-time" by lab test, not visual test. I have only seen blood in my urine twice in 41 years.
 
Seth I agree with your assessment. I wouldn't change a thing if you've been fine right along and this test is the only 4.0. Now if you continue to trend upward, then yeah, cut back by 10% for the week, but at this point, I agree with YOU.

Can we back-up for just a moment?
How would one decrease by 10% for a week?
Would cutting a 5mg in a 1/4 size pill justify 10%?

I'm terrible at math and I have never been able to get "the picture" of decreasing in percentages and Al's chart doesn't help me (which I have).

Is there someone here that wouldn't mind sending me (or posting) a PM explaining a decrease dosage without using percentages?

5mg times 7 days = 35 mg per week. So, 10% (or 1/10th) of that total is 3.5

So, if you wanted to reduce by 10% you would reduce by 3.5 mg.

If you cut a 5mg pill in half that would be 2.5mg

Or if you break it down again to 1/4 that would be a 3.75mg reduction.

Before my dental extractions I was taking 47.5mg for the week. I reduced my dose by 10% to keep it about 2.0 to 2.5 until I'm sure this bleeding issue is over. 47.5-10%=4.75

47.5-4.75= 42.75 I only have 5mg pills to work with, so we'll round that down to 42.5mg

I dose it as such

5mg X 4 days and 7.5 X 3 days for the total of 42.5

Now when I go back to my normal dose, it'll go 7.5 X 5 days and 5 X 2 days which will equal 47.5mg for the week.
 
OK you all, I've just sent out some emails about a too low reading, but after reading the posts about too low, I'm thinking I need advice from everyone. I am supposed to be between 2.5 and 3.5, taking 5mg daily of coumadin. I tested today at 1.7 and have been testing low (2.4 and 2.1) the last 2x I tested. I also had a flu shot last sat. If I go in and get tested on my drs. machines I have to pay a $40 co-pay and my machine has consistently been exactly the same as his. I am lousy at math story problems and cannot figure out the formula for how to adjust for these swings in my #s that I always have. I admire and am in awe of consistency since the only thing I am consistent in is inconsistency. I seriously cannot get this one right, so I'm just groovin' along as best I can and hoping that sooner or later the medical minds discover something that works better than rat poison. Has there been any research on red wine for anti-coagulation? just kidding, but it's a nice thought.
Is it OK or not to take coumadin at a different time? Like what I was thinking was to take 5mg now and take 2.5 tonight when I usually take it....what do you think?
Thanks everyone! I'm glad to be back...have recently lost friends to cancers and have been having my heart pulled on pretty hard. How good it is to see familiar names and faces.

I would take 7.5mg today and again tomorrow night. Then back to 5mg, but next week after you test, I'm sure that you may need to take 6 mg each day (42 mg per week) since you have been steadily dropping.
If I was at 1.7 today, I would try to go get a Lovenox shot.
 
I agree with h\Heartfelt tjat you need to add 2.5 for the next 2 days. In a week you should be in your range. You may need to add 2.5 to your 5mg for one day to remain in your range.
 
Thank you Bina and Herb for your help! One more question, would it be okay to take it earlier, like now, than I normally do at bedtime? I thought I read somewhere that you should take it at about the same time each day, but if you'd go get a shot of Lovenox, maybe I should take it now.

I've been flying blind with this drug ever since my dr's. office stopped helping those of us who are self-monitoring. I'll get the publication to shed some more light on this.

What is the total dose you take for a week normally? Is this dose the one producing the below readings?
