Warfarininfo.com dosing chart

I've seen many of you refer to the excellent information that had, in the past, been available at warfarininfo.com. Unfortunately for those of us who are new to coumadin, that site no longer seems to exist. The wayback machine has an archived copy of it at:

but unfortunately, it doesn't seem to include the fabled 'dosing chart.'

Since it's no longer available for sale, I don't feel bad asking: Can somebody provide me a copy? If we're pretty sure that it'd be OK with Al, I'd also be happy to host a copy of it on a server somewhere for those who come seeking it after me.
