Seven days ago INR = 3.5. The following week I took 75 mg and that dose resulted in an INR of 3.9.

So, what adjustment, if any would be recommended?

Continue with 75 mg or drop down to 73 or 70? A tweak or a nudge?

INR kept a little high on purpose.

I'm happy with the 3.9 as long as it doesn't climb too much.

I wouldn't change a thing. Such a tiny INR increase is nothing to worry about. Eat a large green salad the day your INR is a bit higher. But 3.9 isn't high.
That's all I would do.
 
I agree with Christina, I wouldn't change a thing. If it's still creeping up next week, then tweek or nudge. I take a lot too (70/week right now). I've found that, even with that high a dose, I can add or decrease my dose by 1 or 2 mg and affect a change.

I agree with Christina and Karlynn. Don't tweak or nudge your dose just yet.

BTW: Weather will be getting warmer soon and most people will become more active, which will lead to decreased INRs.
 
I never change my dose unless I am close to 5.0. My INR is usually in the 3.8 to 4.2 area (range 3.0-4.0).
