The date has been set for Wayne's second surgery. January 24th. They will be doing a lot of testing prior to the main event. He is scheduled to have a tricuspid annuloplasty, IF the test results (64 slice) come back with clean arteries. And, IF, once he gets in there, the valve does not have rheumatic involvement, in which case he will replace the valve with a bioprosthetic, even though he has two mechanicals in place. (The reason is that these do better in the tricuspid position) His most recent echo also showed some mitral regurgitation, unquantified, which the 64 slice should be able to do. If not, he will have to have a heart cath, to determine if he has a paravalvular leak that has to be repaired when they are in there. Additionally, they need to verify that his coronary arteries remain clear.
Two or three days later, they will install the pacemaker. (He may also need an ICD, but time will tell on that.) If they have to implant a new valve, then the leads will be placed on the outside of the heart, when they are doing the surgery. If not, which always was our hope, the pacemaker will be implanted 3 days or so after the annuloplasty.
The really good news, is that they are going to try to get in from the side, and not crack his sturnam again.
