Hi everyone

I recall reading through this site a while ago, and seeing some information about using some type of booklet to help calculate the coumadin dose when you test at home.

I am hoping to soon be able to get a home monitor and would like to try to adjust the dose myself since I am out of range so often.

If anyone has suggestions on how well this works, this would really be appreciated.

Thanks

Hi Evelyn,

Al Lodwich has just what you're looking for. Try his site. I think it is warfarininfo.com. Home monitoring is as good as it gets.

Or you may order Al Lodwicks chart from him at:

WARFARIN DOSING ALGORITHMS

This is a single sheet of 110 pound white card stock. It will be folded into thirds. Each section then contains one algorithm in flow sheet format. This is suitable for a clinician's lab-coat pocket, or a doctor's office. It can also be used by those who test their INR at home and adjust their own warfarin doses. The algorithms are titled, "WARFARIN PATIENT ASSESSMENT AND MANAGEMENT PROTOCOL", "INITIATION OF WARFARIN THERAPY FOR AN INPATIENT", "INITIATION OF WARFARIN THERAPY FOR AN OUTPATIENT", "WARFARIN DOSE ADJUSTMENTS BASED ON A DESIRED INR OF 2.0 TO 3.0", WARFARIN DOSE ADJUSTMENTS BASED ON A DESIRED INR OF 2.5 TO 3.5", and "TREATMENT OF INR ABOVE THE THERAPEUTIC RANGE".

Please note the disclaimer, "These algorithms are for guidelines only. They are not intended for any one specific patient. Each patient must be individually treated using the best professional judgment."

Once you start self-dosing - you'll get the hang of how your body responds very quickly. Use the charts as a guide line, be sure to keep you own chart to see changes and before long you'll know what your body will react to changes. Good luck!
