Here's what I remember at this moment:

I think that was actually from my "new" gastroentenologist, Dr. Livstone, after I ended up in the hospital after a botched colonoscopy performed by my "former" gastroentenologist, who refused to consult with my cardiologist and didn't want me to go on Lovenox bridge therapy before the colonoscopy.
