Hello Al, and others:

My husband has been on coumaden for three years. After the initial bout of getting it leveled off, and then going through the same thing post surgery, he has had pretty much a smooth road keeping it within range.

Three weeks ago, however, his INR measured at 3.8, and it hasn;t come down since. (He is a dual valver, so they want it between 2.5 and 3.5. Now, the INR clinic says if it continues next week, a change in meds is in order. Which is fine, but I was wondering what might have caused this. He does have an enlarged liver from the heart disease. Any comments?

As an aisde, but VERY important, I am so sorry to hear the news about the prostate cancer. I know when caught early the success rate is high, but no one wants to go through something like this. You will be included in our prayers. You've been a god send to this sight, and have helped all of us personally on this most important topic.

I would not want to adjust somebody's warfarin down because of an INR of 3.8 unless there was something like a known interacting drug added.

There is much greater risk from a low INR than there is from any INR that is below 5.0.
 
Thanks Al, exactly what I thought.

He seems to tolerate the slightly elevated INR just fine, and I just hate to see them start playing with it.

My husband's range has been 3.0-4.0 for the past 3 years due to a stroke. He likes his INR at the high end of the range, so his INR has been at 3.8 or higher many, many times with no adverse effects. Recently, I heard that some cardiologists in our area are increasing their mitral valve patents' ranges to 3.0-4.0.
