Printable Med and Emergency Contact Card

Hi everyone,

I came across a place where you can enter your prescription info and your emergency contact info and then print and cut out the card to put in your wallet. This comes in handy whenever you need that info for medical procedures, or in case of emergency of course.

I printed mine on cardstock so that it would last longer, and I also put the dosages of each med on it.

It doesn't have a specific place to list your valve info, but I entered mine in the same box as the prescriptions...just in case anyone needs to know.

There is also room for your primary doctor's name and number and then an emergency contact.

I had a dr who gave me a medicine card to fill out. They asked me for it every visit. It was really handy. All of us need to have a list on our person or in our pocket at all times.
