Does anyone have the latest recommendations for taking antibiotics before a scheduled dental procedure? Initially, my PCP gave me an injection as well as an oral dose of Amoxil. Later, the recommended doses were apparently reduced. My past procedure was to take 3000mg of Amoxil one hour before the procedure. Any suggestions?

Funny this should come up, my cardio just gave my a script for
antibiotics,take 4 before any work,

amoxicillin generic for amoxil 500 mg capsules.

while we are on the subject our in house dental member might give me some insight on some dental questions maybe, I have really bad teeth ,my cardio wants them out before i have any surgery, what do i need to do and ask or be cocerned about in getting this done,i'am terrified of dentist and u need put out no problem there of course but should there be concern for my heart
areas.

Hi Jade,

From my experience with root canals and crowns...I was required to go off coumadin for two days and start the antibiotic (Trimox) before procedure at dosage of 2000 mg. I also had 'conscious sedation' which my Cardiologist prefers for me for extensive (invasive) procedures.

For routine cleaning I a again required to be off coumadin for two days and take the same dosage of Trimox. The dosage has changed over the years. The prior dosage was taken before and after any procedure, but has been cut down in recent years.

I was also required to get my INR down for an UltraSoundFineNeedle Biopsy on my thyroid by stopping coumadin. It took 3 days for it to drop to 1.9. It's different for everyone and the Cardiologist has the final say in my case.

It helps to get different people's experiences, so you can present them to your Doc and understand the varied logic.

Thanks for the input ladybug! What I really am looking into is a full dental extraction for dentures,like I was saying earilier my teeth a real bad,really need them out but want to go completely under for this would I might have any trouble with
going under and is this a safe procedure to do in a dental office
enviorment or should it be done as an outpatient in a hospital.
just wondering?


The last dental put under in office and heart rate was a little tacky.

This is Jim's wife, Shirley, a dental hygienist, responding. If at all possible save your teeth. I have many patients who tell me they wished they'd done everything possible to save their teeth sooner. Lower dentures typically don't fit well. Without knowing the state of your teeth I can't give you more specific ideas. Do you know if you have pockets, and what their depths would be?
If you still want them all out, I would see an oral surgeon, a dentist who has advanced training in people with more medical concerns, and specializes in extractions. You'd still need to see a regular dentist for dentures. I especially wanted to respond because you are a whole day older than I am! Good luck!

To this Jim, who is also petrified of dentists, adds: I recently went in for dental work and was premedicated with the standard antibiotic dosages, but I did not stop coumadin. Although the filling turned into a root canal (worst nightmare!), there still wasn't any bleeding.

Just a quick 'add'..Jade, you really need to talk to your Cardiologist as to where/what is safest for you since he/she knows your cardiac risks. The oral surgeon and your Doctor will confer with each other and decide for you!

I agree, too..if you can save those teeth, it's well worth it. Conscious Sedation is GREAT..you are totally 'out'. It's expensive and lots of dental insurances to NOT cover it (mine included..Grrr). The oral surgeon can discuss this with your Cardiologist.

Sorry you are having to deal with TOO many things at once it seems!!!!

Yeah, my teeth are pretty bad,went south about 6yrs ago,started to break off right at the gum line, and with all that ,they just seemed to dissolve from the bottom up , better have them all out at once cause I don't really do good with any type of dental work.Like to have those implants WHOOO them babies are high ,settle with just a day without pain right now.

Hi Jade,
You want to figure out with the oral surgeon and your cardiologist what is best for your teeth. What you are describing sounds like you have some bad bugs working in your mouth. If you can get that fixed up it will also help keep your heart stronger. If there's anything I can do to help or you have any other questions I would be happy to respond. I just went to the Dentist, and 4 500 mg pills of Amoxicillin were the Dose, I took my reqular dose of coumidan, and while it was a bit of a blood letting, it was more do to poor gums and lack of flossing than, the coumidan. Now since I am a regualar flosser, no blood, and happy Gums:D But my teeth will never be as nice as my dogs.

Speaking of...lol...My little Doxie dog had to have a tooth extracted! Of course , no dental insurance for him! $300.00 later and antibiotics he is doing just fine. He had to be 'put out' for the procedure, of course! Geez...are we supposed to Floss our dogs, too? LOL!

Thanks, Folks, for all of the good, if unpleasant, information. I'll stick to my six 500mg caps of Amoxil and stay on my usual Coumadin dosage, at least for getting teeth cleaned. I might consider reducing the Coumadin if something more invasive were involved, but am very reluctant to go off Coumadin for several days.
Good Luck to those with more serious problems. Compared to them, mine seem minor.
 
dental work is horrible. I have three more root canals, and every tooth has at least a cavity in it. Is there something with heart surgery that does this?????

You have had quite a turnout of support. I also had to had teeth pulled before surgery. If your question is why the doctor wants the extractions before surgery is because having it done now will prevent future infection. You may have some infction now in your mouth that the dentist can help you clear up. Bad teeth can cause carditis, which is bad for the heart anyway. Get all the work done now and then relax. I had several teeth pulled and had to go through two dentists, the second actually done soemthing for me. The first one played around for four months and I would up having CHF from the wait. The second had me done with infection and dental work within one month. I am doing better now. I will be glad when spring comes to the midwest. You hang in there honey and you will be alright.

Hello,

I'm currently working as a dental asst. for an oral surgeon/anesthesiologist. We put many people under during dental procedures...It is a safe procedure, If done by a qualified dentist. Since Jan 2001, It has been a law in NY that all patients that go under must be hooked up to a heart monitor. Which makes it much safer. We get a lot of patients who are apprehensive. If you don't have any dentists near you who do it, Check at the hospital, Most hospitals have days when there are dental procedures done for people who need to be more carefully monitored. Thank You Nicole for that info. My husbandhas to have some oral surgery on his bottom (extractions) teeth. He keeps putting it off. He has had CaBGx4 in Aug/2001, and has been told to take care of this. I know he's scared, and won't admit it cause he's one of those 'old tough guys'..u-no? Does your dentist use Conscious Sedation? I found that was great but I am facing another root canal on a tooth that was previousy crowned, but developed a cavity under it. I am putting it off, too. I guess we all dread the dentist!

I think the reason my dentist prefer I go off coumadin for 1-2 days for cleanings is precautionary...just in case bleeding might occur. My Cardiologist agrees. I test INR before and then 3 days after back on regular dose..so far it has been fine. I have my teeth cleaned every 3-4 months.

Thanks again..we all need encouagement to visit our dentist on a regular basis
