Going through a patients chart today, a 49 yr old lady with afib, I saw the notes of the PCP bridging her for routine colonoscopy.

Date Plan
6-21 Last dose of warfarin
6-22 Begin Lovenox 100 mgm subqu
6-25 Last dose of Lovenox
6-26 Nothing
6-27 Colonoscopy Restart both Lovenox and warfarin if OK with GI
 
Pretty simple.
I like to finesse it just a bit by started earlier and determning the start of Lovenox by testing the INR. I also like to be anticoagulated until the last possible minute.

100mg leads me to belive she weighs about what I do.

PS.......please use the expression 49 years young! :D

Your picture makes you look a lot thinner than 220 pounds - the weight for a 100 mg dose of Lovenox (1 mg per 2.2 lbs.)
