New study published in Archives of Internal Medicine calls into question the optimal intensity of oral anticoagulation for patients after myocardial infarction or for those with mechanical valves or afib.
Researchers prospectively studied 4200 patients on warfarin at a Dutch clinic. During 3 years follow up, there were about 300 major hemorrhages or thromboemboli. The lowest incidence of these events occurred at INR's of:
2.5-2.9 mechanical valves
3.0-3.4 afib
3.5-3.9 after MI
The authors support target 3.0 for mechanical valves or afib, and 3.5 for post MI.They welcome additional research to test these levels.
