Medical Data Cards

I just ordered my medical ID bracelet and have been digging through some threads to see what kind of information people put on theirs, etc. I decided to type up a little "card" for my wallet with more detailed information--medical history, doctors, contacts, preferred hospital, stuff of that nature.

Before I get too carried away, I was wondering if anybody has a template for something similar that they wouldn't mind sharing? I am hoping to get some ideas.

I'm not sure of the etiquette of posting attachments on this site, but maybe we could share some examples (after removing pirvacy-sensitive information of course!). I'll gladly share whatever I end up with; it'll be in MS Word format. I can also host templates on my web site if posting here is an issue.

If you've got suggestions that are unique or aren't covered in some of the previous threads, speak up--many of us don't know what type of information may be useful in an emergency...

I don't have a template, but keep it short, to the point, and include as much relavent information as you can think of.
