Is there any possibility that Lovenox could be improved to be able to provide the bridge w/o having to inject it? Like, bind it in a tablet form that easily dissolves and absorbs quickly into the bloodstream.
 
I'm doing my second Lovenox Bridge. It's not as bad as I feared. I have even learned to self inject. (The nurses showed me that I had sufficient 'belly fat that I didn't need to 'pinch' my belly to provide a safe target :( )

Wouldn't that be wonderful!!!!! A pill and not a shot! I am one of the ones who bruises terribly with these shots, no matter what I do (ice, make sure there isn't a droplet on the needle, make sure the needle is perpendicular etc.) and it hurts too!
 
The blood flowing from the stomach all passes through the liver. By the time it comes out of the liver there is nothing left. This is called first-pass metabolism and is the reason that things like Lovenox and insulin cannot be in pill form.

Going to have my shoulder surgery on the 29th of this month and was wondering if the shots are like a pen or the big needles? My PCP will set up for me to go to the hospital if needed to get them. Getting kind of nervous now and this brought this back to mind. Are they painful? How long do you have to take them for after the surgery? Hopefully they won't release me to my coumadin is in line. I think I am getting paranoid about this.

re:lovenox...there are some really good postings on how to use lovenox if you do a search on this site.

Generally speaking you might do well to ice the area where you are going to inject the Lovenox, make sure there are no droplets on the needle itself and inject perpendicularly into the area. You can pinch up some skin and inject there.

It is not a large needle, but not a pen either, it is a regular syringe, and in my case it was pre-filled.

It doesn't seem to hurt everybody, but it did hurt me, a stinging sensation.
I stayed on Lovenox for about 3 days while they brought my INR up with
Warfarin.

Nan, thanks for the reply. My husband is a diabetic so he may be able to help with the shots. I will look for the threads. I have got a feeling that they won't let me go home until I am in range. I don't know what would be worse, the stick every 6 hours with heparin or the Lovenox. Will see Loyola's cardiologist next week and I should have more information.
 
The shots are not painful in themselves since they use short tiny needles, just enough to 'get under your skin'. The more 'belly fat' you have, the better (i.e. I don't have a problem with the shots :rolleyes: )

There may be some 'discomfort' from the injection due to the amount of liquid that is being forced into your layer of fat. I go slowly and take it easy for about an hour afterwards.

Note: This is from someone who was terrified of the thought of self injecting. It's amazing what we can learn to do / accept. Being a Heart Patient isn't for sissies!

I self-injected insulin when I was pregnant and have self-injected Lovenox twice now. I'd say the needles are very similar. No pain really, and just a few small bruises - and I bruise easily. I think I had one rather ugly bruise the first time, but since I was having a laparascopic hysterectomy, it blended in with the bruise around the incision where the inserted the scope.

I also self-injected Penicillin/Ampicillin when I was younger and still on the therapeutic antibiotic treatment after RF. That wasn't near as easy and talk about huge knots! It always helped to drink a glass of wine (or two or three) beforehand!
 
My guess is that your insurance is not going to want to pay for keeping you in the hospital for a week until your INR is in range. The INR is not a significant number until you have been on warfarin for about a week. Sure they can load you with warfarin to make it look like you are in range but that is bogus.

I will know more next week when I see their cardiologist. My pcp said that it can be done at the hospital in special procedures if needed. I was in for 5 days when I had my carpal tunnel surgery. They did tell me that I would be admitted the day before surgery and put on heparin. They were not sure if I would be on the telemantry unit or the Orthopedic. Just want to get this over with and enjoy the holidays.
