To all you Lovenox bridgers...........what dose of Lovenox have you used? My cardio insists on 100 mg for my weight (210). The GE suggested 40 mg. Could my GE be right? What is your experience?

I started slight bleeding 2 days after colonoscopy (24 hours ago). The biopsy site was cauterized real well (photo to prove it). I can't migrate back to Coumadin until the bleeding stops. From previous "bridges" my worst bleeding is when I'm on both Coumadin and Lovenox the day before I stop the Lovenox. Can't get there from here at the rate I'm going.

I was supposed to re-start Comadin tonight. Going into a holiday weekend. So I'll be looking for some guidance from the docs today. In the meantime, I'm back on clear liquid diet (in case they need to go back in, and house (ar)rest.

I know, I know....I would rather bleed than stroke.

Dosing?
The dose of enoxaparin will be different for different patients. Follow your doctor's orders or the directions on the label. The following information includes only the average doses of enoxaparin. If your dose is different, do not change it unless your doctor tells you to do so.

For injection dosage form:

For prevention of deep venous thrombosis (hip or knee replacement surgery):

Adults?30 milligrams (mg) injected under the skin every twelve hours for seven to ten days. Alternatively, for hip replacement surgery, the dose may be 40 mg injected under the skin once a day for three weeks.

Children?Use and dose must be determined by your doctor.

For prevention of deep venous thrombosis (abdominal surgery):

Adults?40 mg injected under the skin once a day for seven to ten days.

Children?Use and dose must be determined by your doctor.

For prevention of coronary arterial thrombosis:

Adults?1 mg per kilogram (kg) (0.45 mg per pound) of body weight injected under the skin every twelve hours for two to eight days.

Children?Use and dose must be determined by your doctor.

For prevention of deep venous thrombosis (in patients with a serious illness who cannot get out of bed)
Adults?40 mg injected under the skin once a day for six to eleven days.

Children?Use and dose must be determined by your doctor.

For treatment of certain types of chest pain and heart attack Same protocol used for AV patients:

Adults?1 mg per kg (2.2 lbs) of body weight every 12 hours injected under the skin for two to eight days. Aspirin 100 to 325 mg orally once a day may also be given.
Children?Use and dose must be determined by your doctor.

Your Cardiologist is correct!

1 mg per 2.2 lbs = 95 mg dose. This coincides with my cardio's instruction. I also see where the GE gets his 40 mg dose from.

Don't worry, I won't change my dose without doctor instruction.

Your Cardio is correct. I've seen Al post it also, but I couldn't find the reference to it.
