Looking for a web site

A few monts ago someone had a website posted on a thread they wrote. I remember it was a New York State and it was about doctors and hospitals and their mortality rates for heart surgeries. I have been trying to find it but can not seem to . Phyllis has tried to help me. But maybe I am explaining it wrong because we both can not find it. I would appreciate it if anyone remembers it and might also know the name of the web site. Thank you for any help

This is what you were looking for. It is a PDF file and sometimes it disappears when you first get to it, but refresh the page and open the PDF file. It lists doctors and hospitals and the stats.
