I went off of coumadin on Friday 6/28 to get ready for a colonoscopy that I had on 7/3. So here was my schedule: 6/27 last dose of coumadin, 6/28 nothing, 6/29 - 7/2 lovenox only, 7/3 - 7/6 lovenox and resumed coumadin. I checked my INR this morning and it was 1.1. My range is 2.5 - 3.5 and my dose has been 39 mg per week. What are your suggestions for my coumadin for the next week and should I continue the lovenox?

You should stay on the Lovenox until you are in range (or at least above 2.0).

As far as dosage - stick with what you were taking prior to coming off if that kept you well in range. Loading doses have never proven to work and usually just cause a yo-yo effect.

What were the recommendations of your coumadin manager?

I echo w/Gina said, "stay on the Lovenox" until you're in range, very important! Once you achieve 2.0 or better go back to your normal range. Do you home test?
 
I concur with Gina's recommendation.

I'm surprised your Coumadin Clinic didn't make this clear during your training session.
 
I do home test. My internist's office said to stay on lovenex once my INR is over 2.5. I thought maybe I should increase the dose from 6 mg to 9 for a few days. I'll follow your advice and continue on lovenex and 6 mg until I'm in range. Thanks all for your advice. Roberta
