O.K the copy I have is a liitle blurred but I give the best results I can. Also three pages of stuff so I'll just give you what I can with out having to type it all.

LV- Non dialated,non hypertrophied LV Symmetric systolic contraction of all LV segments. Normal systolic LV function and EF
RV- Normal systolic RV
LA- Mild dialation
RA- Normal RA
MV- Thickening and/or increased echo density of anterior mitral leaflet with mild prolapse. Thickening and/or increased echo density of posterior mitral leaflet.
Thickening of chordae tendinae
Moderate subvalvular mitral disease
Mild mitral valve regurgitation
AV- Calcified aortic valve and good systolic opening
TV- Normal appearance of tricuspid valve
PV- Normal appearance of pulmonary valve
Pericardium- No significant pericardial effusion
TEE- No known rheumatic fever. Recurrent tonsillitis in childhood. First admission 10/08 for chest pain (GERD) and 6/20/10 for tingling in both hans/arms. Negative CT/MRI of head.
No LVH EF 70%
Thickened MV decrease mobility of posterior leaflets, subvalve diease, mild rheumatic mitral stenosis (MG 3 mmHg) and mild MR
Moderate thickening of left and posterior aortic cusp (fibrosis of nodule of Arantii) and moderate AR
(2-3) from center direct to right.
Mild diffuse thickening of aorta and patchy calcification.
