does anyone know what SAM stands for. It seems that after my mitral valve repair I now have "sams" and I have to have my valve replaced.

SAM = Systoltic Anterior Motion When the ventricles contract, the entire mitral valve apparatus moves forward producing a bulge touching the interventricular septum and interferes with the outflow of the left ventricle. This is tricky business usually diagnosed by echocardiography. In your case the SAM is most likely a rare complication of the repair surgery. In some cases it can be corrected with medicine like beta blockers. Have your cardiologist explain it in simple English and draw a picture of it for you. Also before you decide to undergo surgery, would advise a second opinion.I'd go to Mayo or Cleveland Clinic.

Marty said it perfectly. I had SAM after my MV repair and it practically blocked off the outflow completely and I wound up with the valve replacement. I didn't know about it until after the whole thing was done as it was one LOOOONG surgery session. I did view the intraop transesophageal echo (on tape afterwards, obviously) and you could really see the valve flap over with each beat. I'm certainly not a cardiologist and even I could tell that it wasn't a good thing.

Good. You can't go wrong there. Personally I believe at Mayo they have the best mitral valve surgeons and cardiologists . They will know how to diagnose and cure SAM. Keep us informed. All the best to you. Marty

Paul,

I agree with Marty to get a second opinion. Heck, I met with 2 cardiologists and 2 surgeons before my surgery (I'm an engineer, and question everything  ). If you're interested, I can give you the names of the my cardio & surgeon. In any case, do please keep us informed of your progress. :)
