I just started Cymbalta for depression and anxiety. What interactions has the group learned of with this combo?

Brian - I did some searching on it and didn't find any definitive warnings. Al does not list it on his interactions list at www.warfarinfo.com (and I looked under the generic name). There were a few stories of people experiencing high INR's from the combination, but then someone else reported a low INR after starting Cymbalta.

One story that appeared many times was a woman who's mother was very sick with pneumonia and "nearly died from the Cymbalt/Coumadin interaction causing a high INR." I'm very suspicious of that. If the woman was very sick, the chances are great that she was very dehydrated and it's my guess that the high INR was due mostly to that.

As with starting any new drug, I'd recommend checking your INR every 4 days or so to catch a rising INR or a lowered INR.

WARFARIN SODIUM (in Coumadin) may interact with DULOXETINE (in Cymbalta)

Although the cause of this potential interaction is not clearly understood, duloxetine may increase the anticoagulant effect of warfarin. Warfarin is generally used to prevent your blood from "coagulating" or forming blood clots. When duloxetine and warfarin are used at the same time, your blood may be much less likely to clot and this may increase the risk of excessive bleeding. If these drugs are taken together, your doctor may want to monitor you closely for at least 8 to 10 weeks after therapy with duloxetine is either started or stopped. Blood tests can be used to make sure that you are getting the right amount of warfarin. If you are experiencing problems, it may be necessary to adjust the dose of warfarin. Discuss this potential interaction with your healthcare provider at your next appointment, or sooner if you think you are having problems.
