Hi & welcome. I am only 3 months post op and am still trying to get mine stabilized. That said, I have spent hours reading various threads/posts on this site and investigated other sites to glean information. Here are some of the things I have learned.
Virtually everyone on warfarin has had trouble at one time or another maintaining a stable INR.
Many things can cause your INR to vary - one of the keys to stabilization is consistency - Vitamin K foods, exercise, other medications (internal & external, prescription or OTC).
Herbal supplements can drastically affect your INR.
Hot weather can make your INR rise.
Illness can change your INR.
Testing & dosing adjustments done too frequently can produce a yo-yo effect (mainly due to the length of time it takes for warfarin to get metabolized. Todays dose takes 3-5 days to show up in your system)

You don't say how long you've been on warfarin... there are people on this site much better equipped to give you advice. It would be helpful to them if you gave some details of your daily dosage & INR readings.
