CoaguChek XS System — Cleaning-Related Malfunction of Meter with Potential to Impact Results
Issue___________________________________________________________________________________________
Internal investigations have confirmed that recommended cleaning procedures for the CoaguChek XS system can, in rare circumstances, cause a meter malfunction which could lead to falsely elevated results.
In the vast majority of cases it displays an error message (error 4 or error 8) and no test result is displayed. In rare cases, a falsely elevated test result might be displayed (with or without an error message).
Cleaning with certain cleaning agents has been identified as the root cause, especially if applied near the test strip guide cover (see picture). Liquid entering the meter can cause damage to the meter.
Effect on Results________________________________________________________________________________
There is a remote risk for falsely elevated results. Please consult with your physician if you have questions about your PT/INR results.
Updated Meter Cleaning Instructions_ ____________________________________________________________
Please follow the procedures below to clean your meter. Failure to follow the procedures may cause malfunction of the device.
Cleaning is only required when the meter is visibly dirty.

Hi Scott
Thanks for the heads up.
However when I to the link, I accessed Canada and couldn't find the info you posted about cleaning. Also I can't access the US section. This warning, for some reason, doesn't seem to include Canada.

Is there a date on this notice?

There WAS a reported problem with improper cleaning a few years ago. Roche sent out a bulletin, and the cleaning procedures (and chemicals used to clean the meters) were updated.

I suspect that this problem will be of most importance to clinics and labs that use the XS extensively for testing many patients.

For individual users who home test weekly or even less often, the issue of cleaning the meter is probably very minor.

However, should home testers ever want to clean their meters, it's always a good idea to follow the instructions (or updated instructions) from the meter's manufacturer.

I don't think that there's a problem posting in this area.

The issue - which apparently generated quite a bit of heat at the time it was announced - has pretty much been resolved. As long as users of the XS who actually clean their meters do so with the approved materials and methods, there should be no problem.

(Interestingly, I just received some special wipes for cleaning the InRatio2 meters -- apparently, they may have more recently had issues with meters that were cleaned with the wrong materials)
