This is so dumb... I cannot remember if I took my warfarin today. I meant to take it earlier but cannot for the life of me remember if I did or not. I have a pill box and it is empty. I cannot remember if I took the last dosage in the box yesterday and did not refill it or if I took it this afternoon.
I take 7.5mg on S, T, Th, and S, and 5 mg M, W, and F. My range is 2.0 - 3.0. Last Tuesday I was at 2.2. I have stayed at 2.2 since I came home from the hospital. I am not due to retest until next Tuesday , March 21.
I thought I would call the coumadin clinic tomorrow and ask if I can test Friday morning instead of waiting until Tuesday. I am on coumadin for AFib and since I had a Maze, the surgeon told me my chances of having a stroke are very reduced. I am only in AFib ocasionally, but I can't tell if I am or not. I was not in AFib at Cardiac Rehab yesterday.
 
I would take a 5mg tablet and go back on regular schedule tomorrow. You have been at the low end of your range and 5mg extra should not put you too high. Also, better a little too high than too low. I would test again on Friday to make sure you have not dropped too far.
You should also get a "days of the week" pill box. That way you will know if you have taken a dose on any given day. Get in the habit of refilling on Sunday and you should be set.
Keep in mind that this is a suggestion as I am not a doctor or pharmacist.;)

Thanks for your suggestion. I do have a day of the week pill box and I do my other meds by the week beginning on Sunday. BUT since I usually test for INR on Tuesday, I have been doing the warfarin from Tuesday to Tuesday. I will have to start that one on Sunday also.
 
I agree with Geebee. Taking 5 now to be safe, since you are on the low end of your range, will not do as much "damage" as not taking a dose and wind up going too low.
 
I did that once too. I took 1/2 tablet (2.5mg) just to be safe. I home test every week so that wasn't a problem. I use the weekly pill box and always fill it on Sunday so I don't do the same thing again.

It will be almost meaningless to test on Friday. If you did forget a dose, then you will probably be low on Friday. If you took an extra half, you will probably high. Either way you are tempting the warfarin manager to adjust your dose inappropriately. This will almost surely lead to you being out of range at the next visit. If missing 1 dose were a sure ticket to disaster, then you are in big trouble. Just bite the bullet and say that mistakes happen and people live through them. Get tested again next week as planned. I doubt there is anyone who has taken warfarin for more than 6 months who hasn't missed a dose - maybe they did not even notice it.

OMG - just as I typed this it occurred to me that I didn't shoot up my mid-day insulin fix today!!! And I'm a pharmacist!!!

as most civilized folks are in bed...........and these guys are the experts anyway, but we keep a calendar and check off when Katie gets her coumadin dose and whether it was 2.5 or 3, since we alternate. Keep her INR draws on there, too. Just a thought. Hugs. J.

I fill all of my daily pill compartments up to the day of my next INR.
It helps remind me to test.

Also, I wear a med-alert necklace that I take off at night only after taking my night meds (Coumadin in that round).

It's kind like wearing a belt and suspenders.:D

We mark off a calendar for our dog's 2x daily medicine. Unfortunately, it works less as a reminder, but more as a record of our poor memory. :rolleyes:
