Does between 2.0 and 3.0 sound right for a recent (Feb. 12) mitral valve repair?

I was started on coumadin (or rather warfarin) on the 14th at the hospital. I came home on the 17th. I am now dealing with the Kaiser Anticoagulant Clinic -- between 2.0 and 3.0 is where they say I should be. They had me go to the lab for a blood draw on the 19th, and called me the same day with the results. They said my INR was 1.8, three points higher than it was at the hospital. It is still below the range they want it at, but they say it should not go up too fast.

The prescription I brought back from the hospital is for 2.5 mg. of warfarin a day. That is still supposed to be my dosage except for Sunday when I am supposed to take a half tablet, 1.25 mg. On Monday the home health nurse is coming by and she will do another blood draw; based on that they will again review what the dosage should be. I am supposed to call them for any number of conditions or events that might affect INR (infection, flu, vomiting, diarrhrea, fever, fluid retention, etc.), major diet changes, changes in activity levels, medication, etc., etc. They also told me not to worry too much about diet, just try to be as consistent as possible, and they will work the anti-coagulant around it. Right now, just out of the hospital, I couldn't worry about diet if I tried. Just finding things I want to eat is a bit of a struggle.

My insurance is through Kaiser -- it is a non-profit HMO with its own drs., hospitals, clinics, etc. -- so at this point I have little choice in who to rely on for my anti-coagulant therapy.

Marge, Are you fibrillating? I'm not sure about warfarin indication after a repair if you are in normal sinus rythym. Have they told you how long you are to take the warfarin?As to Kaiser. I work for mid Atlantic KP as a radiologist. Our warfarin program is run by PHD pharmacists with oversight by the cardiologists and hematologists. They are very good.
I self regulate with a Coaguchek so I don't use the Kaiser lab but I think it is very accurate though I have not used it in over 4 years..I have a mitral mechanical St. Jude and was told the standard was INR 2.5 to 3.5 but I try to keep mine closer to 3.5. With your INR of 1.8 up from 1.5 I would raise the weekly dose about 15%. Then test and see where you are. We like the 10-15 % rule here adjusting up or down as the case may be. Think in terms of weekly dose changes and then divide by 7 to get your daily dose.

Did you have a repair of your own valve that you were born with?
If so, they may just keep you on warfarin for a little while post-op.

If there is anybody who should get warfarin right, it would be Kaiser. They have about 6,000 people in the Denver area on warfarin. Their pharmacists there often publish papers on warfarin.
 
Marty, I had one brief episode of A-fib at the hospital, I think it was the second (maybe third) day after the surgery, otherwise I have been in NSR both before and since the operation. But I don't think they have me on warfarin for the A-fib. I was told it was standard procedure for the Cosgrove ring I got as part of my valve repair. (Something about needing it while the ring is bonding.) I'm supposed to be on it for three months only.

It's good to hear from both you and Al that I ought to be able to rely on the Kaiser coumadin clinic. I've had a good experience with Kaiser throughout this stressful period -- the exception being my little disagreement with them about cardiac rehab policies.
 
Thanks, Marge, I learned something. I had heard there were some repairs in NSR that they kept on warfarin for a while but didn't know why. About rehab. It is so important, I wish you would join a health club, buy Dr. Coopers book( see my little Q&A in "links") and do it on your own. You can start for a week or two at the nearest hospital with a rehab unit and pay your own way.
I paid $400 for three months at my hospital rehab unit and then went to my club. I'm still at it 3 to 4 tmes a week.
