Hello all. I was just wondering if there is any variation in what is prescribed for prophylactic antibiotic treatment prior to general procedures to prevent bacterial endocarditis. I am still pre-AVR and the regimine has changed over the years. Since I am allergic to many antibiotics, I'm prescribed 600 mg of Clindamycin HCL one hour prior to dental procedures. And that's all. If I were using penicillin (amoxycillin) would it be 2000 mg prior to the dental work?

Years ago (and with stomach gnawing erythromycin) I was to take a double dose 1 hour prior, and then several doses every 6 hours thereafter.

Does this practice continue after one's surgery??

Here are the guidlines given to my husband post-op on 12/2003.
Standard general:Amoxicillin, 2 gms 1 hour before procedure
unable to take oral medications: Ampicillin 2gms IM or IV 30 minutes before procedure

allergic to Penicillin: Cindamycin 600mg 1 hour before
or
Cephalexin or Cefadroxil 2 gms 1 hour before procedure
or
Azithromycin or Clarithromycin, 500 mg before procedure

allergic to Penicillin and unable to take oral medication:
Cindamycin, 600mg 30 minutes IV before procedure
or
Cefazolin 1gm IV 30 minutes before procedure

I take 2000mg of Amoxicillin one hour before procedure.
Yes this continues after valve surgery.
Some years back it was anti-biotics before and after those procedures, but that changed a few years back.
The 2000mg Amoxicillin is probably the most common given today.
 
Just had the six month checkup last week and I am allergic to penicillin and erythromycin....so I am taking the 600 mg of clindamycin and haven't had a problem(knock on wood). They found a small cavity so the just happened to have an appointment the next morning so I got to take another 600 mg of clindamycin. Never really bothers me just the taste it leaves in your mouth, like you have been sucking on a penny....yuck !!! :mad: It sure is alot easier than it used to be...I remember taking it two days before and two days after a cleaning or filling
 
I'm on the 600 mg Clindamycin, too. Usually no problems, but if I haven't eaten much before I take it, I may have a "gurgly" stomach for a day after taking it. Sure beats the alternative, though. . .
 
Hello

For dental procedures I recommend cephalexin and take 8 250 mg 1 hour prior to the procedure.

Doesn't upset my stomach and I have used this drug for several years after an adverse reaction to clindomycin.
