After a real insurance hastle a referal to another podiatrist, I was FINALLY seen for my ingrown toenail and the side needs to be removed.

THIS podiatrist is OK with remaining anticoagulated and my Coumadin Clinic has 'trained' the new CRNP who recommends an INR between 2.0 and 3.0 (think I'll shoot for 2.5). He uses a 'toe turnicut' to minimize bleeding and cauterizes as necessary.

Actually, the podiatrist was all set to do the procedure today (last INR 3.5) until I suddenly realized I was NOT premedicated. THEN the podiatrist suggested an antibiotic more appropriate for foot bacteria than mouth bacteria and went through a littany of antibiotics until we found one that I do not react to, namely Keflex. He suggested a full course starting two days before the procedure.

Does anyone know if there is a standard premedication protocol for 'skin' procedures? If so, what are the options? (i.e. what antibiotics, dose, and duration?)

I take Keflex as a pre-med for dental and have taken it for in-grown toenail surgery. I am allergic to just about everything else.
Keflex has worked very well even when my toenail once got infected.
I usually do the same thing as dental - 2grm before and 500mg at 4 & 8 hours after. The time my toe got infected, I went on the 10 day regimen.

My podiatrist had me take an anti-biotic for several days prior to the surgery.
I can't remember which one it was.
It certainly is a good precaution.
