It's time for YOU to Take Charge of Your INR Management

After being Out of Range twice in just a few weeks, it's clear that your INR is NOT being managed properly.

Now that you have Graduated from High School and are 17 years of age, YOU need to be taking responsibility for your INR Management, whether you do self testing / dosing or continue to use a Nurse Manager. Once you leave home for school or employment opportunities, YOU will be the 'One in Charge'.

Either way, YOU need to understand that when your INR is Below 2.0, you are at HIGH RISK of a STROKE. YOU need to understand that when your INR is Above 5.0 you are at increased Risk of Bleeding (gums, nose, internal) that is 7 times the usual bleeding risk at an INR of 6.0

To maintain your proper range, YOU need to be tested regularly (at least once every 4 weeks or every 1 or 2 weeks if your INR is unstable) AND YOU must insist on getting your INR Results the Same Day or at the latest, the Next Morning so that you can make the appropriate change while the Test Result is still valid.

Making changes on 2 or 3 day old Test Data almost GUARANTEES that your INR is going to Yo-Yo as you have seen in the last 4 weeks.

It's time for you to UNDERSTAND how Coumadin works and how to properly manage it. You can 'learn the ropes' from www.warfarinfo.com or a properly trained INR manager (your Nurses seem to NOT be properly trained, unfortunately).

I hope you will take this Responsibility Seriously.
It literally could be a matter of Life or Death (or a life as a Stroke Victim).
