I just saw AL Lodwick's Warfarin / Coumadin Dosing Guide.

It is a well organized set of easy to read and easy to follow flow charts covering several different ranges of Starting INR's and Target INR's.

I heartily recommend it to anyone who wants to understand how to 'get to where you want to go from where you are' in terms of INR.

Hey Al,
I have been using the Warfarin Chart for awhile now and it is Great!
Took it with me to my last blood draw which I have every six months and the nurse said it looked good to her. I only have to call in when I am way out of range.

Glad to hear that you are doing so well.

I got a call today to go to a hospital in another state asking me to train their pharmacists on how to manage patients in the hospital. I will be there 5 days. That will be interesting.
