Matt came home from England with a 1.5 INR!!!! ohhhhhhhhhhhhhhhhhhh! the drama of what to do.
 
Well do you know what to do or are you asking? If your asking, what dose is he taking now and can you give us the schedule?

He takes 4 then 4 1/2 repeated.... and yes, I would love some help!

His range is 2.5 - 3.5 His Inr is usually 2.9 - 3.1

He is also complaining of a migraine but that confuses me as his migraines are usually with a higher INR???
 
I'd bump him to 5mg daily and retest in a week. Is he eating something with hidden Vit K? Excercise much? Something really different from ordinary? Meds?

No the only change would be what he ate while in England and how little sleep he had?? He took his Coumadin.
I did have him take 5mg last night but wasn't sure how long to stay with it.

Below is the chart for 2.5 - 3.5.

Migraine could be stress, travel, lack of good rest. But you should call your doctor and run it by him/her. What was his INR prior to going to England? If it was on the high side of the range, then chances of his INR being low for very long are slim. Did he say whether he took all his doses?

If it were me, I would do what the chart recommends. I'd take an extra dose (I'd probably take the extra dose right away), increase my dose by 10% and test in 4 days. I'd check in 4 days to make sure it's not looking like it's going to go too high (and the reason I'd do the 10% increase instead of the 20% because travel can mess with your INR sometimes.) and also to make sure it's going up at a rate I'm comfortable with. I'm stressing that this is what I would do. I know my body and know how it reacts to dosage adjustments. Your son may be different.

If it's travel that made it drop, then you'd need to watch any dose increase because it may cause him to go too high once his body adjusts back to a non-travel routine.
 
Looks like Ross and I have a different plan of attack. That basically end up in the same mathematical ballpark after a week.

I suggested the extra dose and increase by 10%. Ross is suggesting a ~20% increase. I do the extra dose because my INR is hard to bump up when it's low and I've found I have to give it the extra push.

If your son's INR reacts quickly, then Ross' suggestion would probably be the way to go.

The other issue is what pill sizes do you have?

Thanks for the chart! I did email the dr concerning the headache. Waiting to hear back. His INR was perfect, 3.0 the day he left.

Matt has a hard time with Coumadin. If we bump it up he usually takes forever to increase the INR but once he does, he goes to high. Confusing!

I don't want to wait to increase it as it is so low. His pills are 1, 2 and 4 mgs.
 
Jeanne - does Matt home test? That's what has kept me from having the wide swings. The rule of thumb is not to test more than once a week. But when I'm dealing with a low INR that is causing me to make a fairly significant change to my dose, I test a few times a week to make sure I'm not heading back past the high end of the range. Generally, too many dosage changes make you swing all over the place. But I've found that with a low INR on the way back up, I can tweak the dose a bit to keep from going too high if I see it headed that way.

He does home monitor. Tests once a week. This week I will make sure he does every 3 days to stay on top of the increase.

No help whatsoever as I am still in the "it's a crapshoot" stage, but I'd a been on here asking, too. Glad to hear that Matt is home safe, though, and prayers that you get that INR up soon. So when are you going to regale us with the stories? Much love. J.
 
Well at least he probably did not indulge in any alcoholic beverages over there. But, it seems unlikely that his vegetable consumption skyrocketed either. Did you consider the possibility that with the time changes, long flight etc that he may actually have missed a dose?

Yes that is what I have decided. But his INR only went up to 1.7 with the increase. The increase was 5mg with 4.5 every night which usually increases it too much, but this time not at all...

Increase again??
 
My husband has the same problem. He was 1.5 last week and we bumped his dose up 20 % and he stayed away from the greens. This week he tested 1.5 again. Now what do we do also?
