How much warfarin do you take to get to an inr of 2.5 to 3.5?

Thank you. I communicated it on a weekly basis as that seemed to be what most people seem to say here. I don't think of it that way. I am a little encouraged you have had to take 14 a day. This 15+ and having an inr at 1.4 is very unusual I think.

How much warfarin do you take to get to an inr of 2.5 to 3.5?

I have had to take what I think is a shocking amount of warfarin for almost 3 years now, about 102 mg a week on average. Its shot up to 115 a few times and been as low as 95. What compelled me to post this though, is I decided to stop all alcohol about 5 weeks ago now, and expected that might...

Joint Statement on Aortic Valve Replacement in Low-Risk Patients Issued by Society of Thoracic Surgeons and European Association for Cardiothoracic Su

Medtronic and Edwards have made some excellent tavi valves. It's also probable that most folks can get a second one inside the first one. Already, this hypothetically looks like savr 12 years, tavi 10 years, tavi 10 years. So one open heart, no anti coagulation in theory, and you get 30+ years...

Joint Statement on Aortic Valve Replacement in Low-Risk Patients Issued by Society of Thoracic Surgeons and European Association for Cardiothoracic Su

Sure, if you didn't already know this I would be stunned. We got here because urban surgeons in busy medical centers are very confident doing open heart surgery on patients up to 75 and a little past that who are healthy for their age. Indeed, they are confident doing a third open heart at that...

Afib 6.75 years after OHS AVR??!!! WTF!!!??

I agree with all of this but want to add that Edwards is pushing some of this agenda to do tavr's earlier on folks who are in the upper moderate range. Perhaps they are exactly right but they may benefit from this more than any other company. I lived well hanging at the edge of severe for around...

    Coagcheck Machine post surgery

    12 years would be awesome.

    Coagcheck Machine post surgery

I have to say I would do something like that too, but I figure Roche would change the strips to make very old units unuseable.

    Coagcheck Machine post surgery

Does anyone have an idea how long a CoaguChek XS machine normally lasts? Mines coming up on 2 years so I expect many years out of it but wonder what is realistic?

    Exercise and Tissue Valve

When I reread my comment I realized it was unclear. The odd part was that the 30 year study showed better results than the 25 year. But within the margin so its meaningless. And I meant that basically if we have the same results at 25 and 30 years of around 90%, than it appears that there is...

    Exercise and Tissue Valve

Sure, not exactly as what I want is average time to reoperation, but very close. No great way to wag from this but if 92% are alive and not having been reoperated, that would be a great thing to tell patients, instead of "it will last a lifetime." " You have a 92% chance of it lasting at least...

    Exercise and Tissue Valve

It boggles the mind that a large study has yet to be done that shows average time to reoperation for mechanical aortic valves placed in patients under 40. It would be useful to use patients with no comorbidities. It would be useful if only patients compliant on anti coagulants were included. Is...

INR - How low should you go?

****, you can have a stroke from either a bleed or a clot. A lack of o2 from a clot, or a hemorrhagic stroke(bleed). This is most of the reason for an inr range, neither too thin or too thick. All the best to you!

    Exercise and Tissue Valve

I randomly chose this post to respond on the issue of bio valves wearing out from lots of exercise. The articles you posted are good. I ran much more after my second valve, much more and a dozen half marathons, and it failed about 20% faster. I'm also sure I had clots on that valve and probably...

The Norwegian researchers surveyed 245 patients with a mechanical aortic heart valve. The participants' average age was 60. 87% of men and 75% of wome

But when you build things aren't you afraid you are going to cut yourself and die from bleeding out shortly thereafter? :)

BAV facing OHS in Oct (AVR and aorta graft)

Yes, I knew at the time I was going to be setting an example of some sort. My family is very long lived. My mom just passed at 96 and I spent time the past few days with her 92 and 90 year old siblings. Same thing on my dads side. my brothers are alive with no issues in their 70's. I literally...

BAV facing OHS in Oct (AVR and aorta graft)

It's a very sensible approach to do a bio valve and then mechanical later. Those of us who have had to resort to warfarin know how easy it is once you are self monitoring. Of course the whole situation isn't ideal, but we have to make the best of it. I myself may have allowed some bad decisions...

    Valve Selection

Some clarifications. Are you 60 or were 60 in 2019? Its a first surgery? High volume surgery center I hope, either way? If you are 60, and it's the first procedure, and you expect to live into late 80's, I would not do a tavr. You should get through a savr just fine. If you are 64, it's a...

Highest INR you have heard of or experienced?

Highest I have had so far is a 4.9. I check weekly so this was really odd. I took a day off warfarin and rechecked and was at 3.9. Home testing. The 4.9 was likely correct. It worried me for sure but later reading items here from pellicle has me thinking its fine to get up to 4 for sure, though...

    Valve Selection

I believe that oac does indeed decrease/slow svd. Any echo technician has known for 20 and more years that velocities are decreased on folks taking blood thinners. The valve isnt taking as much of a beating to begin with. Then, the anti coagulants dramatically decrease the possibility of clots...

    pre-op numbers e.g. AVA

In my case, I am not certain, but similar to yours using various equations. My cardio group focused mostly on mean gradients and peak velocity.
