Finally set up with home monitoring

Hi all, Thanks to the help and guidance of all the great people here, I finally got mom set up with home testing through Roche. The nurse came out Friday for the training and we'll do weekly testing. Apparently the Coaguchex app only exists for apple and we run all android but I can submit...

    INR above 8.0????

Still wish someone of her doctors or the people at the coag clinic could give me some idea of what the heck happened (especially since I strongly thing it's tied to the anion gap going high). I know it can sometimes be caused by high lactic acid and she has been using her legs more with the PT...

    INR above 8.0????

Got the finger stick done again today, 2.0 so we're going from MWF at 5mg and the rest at 2.5mg to MWF and Saturday at 5 and the other 3 days at 2.5 (so +10% overall weekly dose) and test again in a week. Also got the paperwork filled out for the home kit so I have to send that in next week and...

    INR above 8.0????

Not sure what you mean? Trust the tests at the lab or the 2.5/5 alternating?

    INR above 8.0????

Well we got her tested yesterday, after no warfarin Monday, Tuesday, Wednesday and Thursday, her INR Was 1.8. They looked at all her bloodwork again and have no explanation for the spike. The anion gap I feel must be tied to it somehow but the potassium and chloride on their own were all fine...

    INR above 8.0????

Also called a nursing home. But she was in a short stay bed since she was just there for the rehab, not long term care. From the nursing/med management side tho, the same. There is definitely something odd going on here. The home health nurse was out yesterday and she's baffled too. Mom has...

    INR above 8.0????

We didn't do the vitamin K tablets (yet) because I was waiting to hear the actual reading from the blood draw tho I did have her eat green beans last nite to hopefully start to get things notched down. Here are the results recently (11/19 - 11/28 was hospital, 11/29 - 12/8 was skilled facility...

    INR above 8.0????

Just noticed her anion gap is "high" at 18 (vs the normal range of 7 to 15). All test in the past had been 7 or 8 so I'm looking into what that means since I know Potassium is one of the "unmeasured ions" that's part of the gap.

    INR above 8.0????

Just heard from the lab and the results got posted to the portal. Vein draw was 8.22 on the INR so it's definitely not an issue with the strips. on the CBC, everything is in the normal range sans the WBC which is only slightly high and expected given the cold I think. They also did a CMP to look...

    INR above 8.0????

Superman - nope, one juice glass worth / day. She can't even reach the glasses let alone pour anything for herself pellicle and dick0236 thanks for the added info. That's very interesting about the lasix too. She's been back on it for about a week but still could be "ramping up" in her system...

    INR above 8.0????

Pellicle - My guess is since you translated ginger ale to ginger beer you're in the UK :) This isn't ale like you'd think of it, it's a soda/pop. She actually doesn't drink alcohol at all so at least we don't have that factor to content with too. We don't have a home meter (yet) so the anti-coag...

    INR above 8.0????

And re: the abx they used, I questioned the hospital pharmacist on the floor about it since I knew that to be one that affects INR (like doxy) and she's like oh that's not one we are generally concerned about. Um ok... what do I know, I'm a database administrator.... but since they were testing...

    INR above 8.0????

Yep she's been off the abx well over 2 weeks now. She drinks a juice glass of either cranberry or tomato juice every morning and has for many years so there's nothing different there (and as an aside, I wouldn't let her take her normal cranberry juice to stave off the UTIs since there's so...

    INR above 8.0????

So my mother who's been on coumadin and pretty darn stable for 35 years just went for her finger stick (still working on the home kit) and it read 8 (which is the max of the machine apparently). Her caregiver was with her so they called me. She retested in another finger, same. They're going to...

How do I get mom started with home testing?

I found this article that shows that she should be covered even with the native valve http://www.prnewswire.com/news-releases/medicare-announces-expanded-coverage-for-warfarin-patients-monitoring-clotting-time-at-home-57026082.html so that's good news. I'll see what the info packet says and then...

How do I get mom started with home testing?

Huh I didn't think to ask Mark at Roche when I talked to them. He's going to send me the information packet to get started. If I went on eBay and got things, I don't know if her clinic would just let me call in the results and do the dosing that way (I know her doctors wouldn't want me self...

How do I get mom started with home testing?

Thanks ****!! That's extremely helpful. The place she goes to says once a month assuming she's in range 3 tests in a row but told me that some services require 2x / month at least. Is that how Coaguchek is? I assume I could just buy more strips on my own if I wanted to test say weekly and just...

How do I get mom started with home testing?

Hi all, Been a while since I wrote but mom's ticker is still going strong, she's developed other health issues but we're plugging along. I took her warfarin management away from her GP after a discussion with her cardio. He has always supported a finger stick while her GP insisted on blood...
